Stalker's Flurry is a Class Feature in Baldur's Gate 3. Features provide unique abilities that are inherent to a Character's Race, Class, Background selected. These may vary greatly, from allowing you to equip certain Weapons or pieces of Armor with Proficiency, to add your Proficiency Bonus to certain Saving Throws.

At the level when all martial classes should be getting fighter's level 11 improved extra attack, Gloom Stalker gets the ability to make a third attack... only if one of their first two miss. This is supposed to be the level when all martial classes gain a permanent increase to their at-will damage output. Unfortunately, WOTC screwed this up horribly and only gave Paladin and Fighter decent features at this level. Seems Larian didn't go about fixing this problem.
In defence of this feature, this will actually come up. If you have a 65% chance to hit with your two attacks, you can expect to miss one of them about 58% of the time. In this (admittedly non-optimal scenario) you can almost read this feature as, "gain a third attack per turn 58% of the time," which is... well... fine, I guess. B-, but level 11 is supposed to be one of the biggest power spikes a class gets. This is okay, but it's hardly in anything compared to Extra Attack (2) and Improved Divine Smite.
Things is, unlike in Tabletop its so easy to get ridiculous accuracy on your attacks (stacking bonuses, Risky Ring, etc) that I this feature isn't enormously powerful. It raises your expected minimum damage output but not your expected maximum damage output. My Astarion Gloom Stalker had an 88% chance to hit on basically all of his attacks by this point. I would have to deliberately make him less accurate and weaker if I wanted this feature to be more useful.
WOTC ****ed up so badly with the level 11 martial features. Extra attack (2) should be standard at level 11, not unique to fighter. Paladin is the only other class that gets a properly scaled level 11 feature. Ranger has it bad by somehow Barbarian has it worse. It's one of the biggest problems with T3 and T4 play. Most of the martial classes just aren't properly designed at these level ranges. Literally just multiclass after level 8 or respec your character into something else at this point.

Can it trigger twice if both attacks miss, or only once per round?

This is pretty good in conjunction with sharpshooter skill, if you have shot that's practically guaranteed, you can still attempt it with the -5 chances to hit and if it misses, it's no biggie.
