Staff of Crones is one of the Simple Two-Handed Melee Weapons in Baldur's Gate 3. In BG3, each type of weapon has different ranges, damages, and other features (Finesse, Versatile, Dippable, etc). Characters need to master certain Proficiency before using a weapon, and sometimes gain a special Actions while holding it.
Live worms wriggle beneath the crumbling surface of this half-rotten staff. Merely touching it fills you with a wave of unbearable nausea and a strange longing, as if it's calling for its like.

As of patch 4.1.1.2154614 the item works as follows:
1) Staff grants unlimited uses of Ray of Sickness (2d8 + poisoned condition). CON save by the enemy halves the damage and negates poisoned effect.
2) Each use of Ray of Sickness granted by this item deals 2d4 poison damage (half as much) to the caster. Regardless of whether you hit your target or not, it saved or not, you get 2d4 poison damage. You never get poisoned by it.
3) Interraction with ring of poison resistance: as intended it reduces the damage caster takes by a half.
4) Interraction with robe of poisoner: adds additional separate damage dice 1d4 (poison) to both outgoing damage and incoming damage.
Possible early game build to negate self-harming component completely: use ring of posion resistance, play either strongheart halfling or dwarf, use poison resistance potion (lasts until long rest). Free RoS's without downsides. Keep in mind there are plenty of enemies that are resistant or immune to poison. Probably the most common damage type for which enemies have resistance in EA.

Poisoner's Robe + Ring of Poison Resistance. Should essentially make this staff a really good damage dealing cantrip for a wizard or warlock once all their spells are used.
2d8 + 1d4 (From robes of poisoner) = 3-20 posion damage + the enemy is poisoned if they fail the saving throw (Which impacts their chance to hit you which is a good debuff).
You should only take 1d4 damage poison with the Ring of Poison Resistance.
But the item seems to be bugged and not apply the effects properly.

It seems that the caster also takes poison damage when the ability of the staff is used
This Staff isn't needed anymore for Mayrinas Quest. Instead theres another Wand on the table.
