Sickle of BOOOAL is one of the Simple Melee Weapons in Baldur's Gate 3. In BG3, each type of weapon has different ranges, damages, and other features (Finesse, Versatile, Dippable, etc). If you leave at least one follower of BOOOAL alive, the sickle will have the blessing.
Many people report that id lost after long rest. So, in order to avoid that, make sure to put the sickle on the chest of your camp before long rest. You can pick up after the long rest and will maintain the blessing.
Pretty good since is one of the only 2 sickles with 2 d4 for dmg.

Paladin specific dialogue allows you to fight Booal WITH the help of the Kuo-Toa.

The Dark Urge could use the "I kill in my sleep" dialogue option to skip the combat, be awarded this sickle and get the EXP all the same like you do with other dice roll dialogues that successfully skip combat

i dont know if you get a generic sickle of 2d4 when you kill him but i heat metaled him then killed a picked up a generic sickle with the same 2d4 slashing on it then looted the real one of the chief so i now have to 2d4 slashing sickles

Guys, the blessing on the sickle is just a hoax. It's only on the sickle, do nothing when you equip it, and will lost the blessing on the sickle after a long rest.
Since those fish priest will keep the killing no matter how you tried to convince him. You may as well kill them all for the exp.

I tried to fight Boooal and the fish folks all by none lethal attack. Although they are all alive, and I get to loot the sickle. The sickle has no blessing on it.
Later I tried to share the power with Boooal and got the blessed sickle. Equipping the blessed sickle will not show the buff on the character like Lovitar's Love.
The only way I get to see the buff on me is to sacrifice a companion.

She just convinced the fish folk to attack him.
Just to add to this - if you manage to reveal BOOOAL's true form, and choose the option "I'll just kill you and take it for myself" when he offers to share power, you will enter combat only against BOOOAL (avoiding having to fight all the fish).
After you kill him, you can pickpocket the Sickle off of the cult leader.

For the sacrifice part, you get a permanent blessing of having advantage against bleeding targets - it sucks.

Well, the optimal way to solve this for me has been to face him with all these weird fish, but using non-lethal weapons.
It allows you to get the experience of defeating them, but since they are still alive, the cult remains, which means that when you loot the weapon of the fish that governs the cult, it still maintains the effect.

You do get the Sickle of Boooal if you pass two dice checks - arcana and religion. After that you share the power and talk to him and he hands over his sickle.

In the full release you dont get the sickle, even if you disarm him, you'll only get a normal grey one.

Boooal's benediction seems to be removed from the sickle after a certain event (might be long rest)
