How to unlock Frenzy
Frenzy can be unlocked by the following classes:
GUIDE TO FRENZY IN BALDUR'S GATE 3
Choosing the Berserker path at 3rd Level will allow a Barbarian to turn his rage into Frenzy. During this period, you can make a single melee weapon attack to perform as a Bonus Action on each of your turns, after activating it. Contrary to the 5E rules, the Berserker will not gain a level of Exhaustion.
HOW DOES FRENZY WORK IN BG3?
Among the 3 subclasses of Barbarians, the Berserker is the only subclass that can activate Frenzy. This transforms their rage into something more powerful and allows them to release their fury and unquenchable bloodthirst. Other properties of rage will still apply while Frenzy is active including the duration of Rage/Frenzy which lasts for 10 turns. This action, however, will end early if you haven't attacked a creature or taken damage since the last turn, and the Barbarian is also unable to cast any spells.
Additionally, the Berserker will also gain the Frenzied Throw and Enraged throw during this period, allowing them to fatally damage hostile creatures.
LIMITATIONS OF FRENZY IN BALDUR'S GATE 3
At the time of posting, LarianStudios announced that the Exhaustion will not be activated for the Berserker when he leaves the Frenzied mode. In the rules of DnD 5e, this drawback is what's stopping the Berserker from using this powerful action. Since this has been removed, the Berserker could prove to be really strong in battle in the full launch of Baldur's Gate 3.

Both Enraged Throw and the frenzy bonus action version of Improvised Weapon Attack do not cause strain as of the September 22nd, 2023 patch.

For anyone wondering, the bonus action version of Improvised Melee Attack only appeara while you are Frenzied. And it also does not cause strain.
Just look to at it your your bar after you rage: you should see both the Action and Bonis action versions.

Is this currently bugged? I am getting Frenzied Strain debuff stack for when I used the improvised melee weapon and when I cast the Enraged Throw with Karlach. It doesn't mention Frenzied Strain for those abilities, it only mentions a stack of frenzied strain for the Frenzied Strike, so I assume this is a bug. This makes Frenzy much weaker, because after a few turns, the Attack Roll debuff from Frenzied Strain really becomes overbearing.

Frenzied Strike and Enraged Throw are OK, but the real reward is the Improvised Weapon Attack as a bonus action every turn. When you pick up a creature and slam it back down (on the ground or other enemies), there's a good chance for it to be knocked Prone. Then you have your Action free for melee attacks with advantage.

One big difference between frenzy VS rage is that frenzy is missing the clause where it says you cannot active with heavy armor equipped.
If you dipped into three Barbarian from Fighter or Paladin, and specifically took Berserker, it looks like you could frenzy while wearing heavy armor (at least on paper here).

They added a stacking minus one to hit debuff every time you use the bonus action, so now berserker is actually worse than it was in pen and paper d&d. At least on table top one level of exhaustion only affected skill so you could use it once a day with minimal downsides. This is truly crap and tells me the developers have no actual understanding of D&D mechanics. They should have either removed the debug entirely or just made it give you a minus one after your rage ends once for each fight, not stacking for every round you use it. As is their is no reason to take berserker over wild heart ever.

Frenzy raplaces Rage and no, it does not have 5e drawbacks, its a straight upgrade with no downsides

does the bg3 version of frenzy not give exhaustion like the 5e version?

Does Frenzy supersede Rage ? When I get Frenzy at level 3, the Rage icon disappears.
They removed being able to Frenzy with heavy armor. Now your rage gets impeded....
