Baldur's Gate 3 House of Healing Walkthrough

You can find the entrance to the House of Healing at the end of the west path from the Mason’s Guild, blocked by an oak door with a lockpicking DC of 10. Inside should be a Key on the table. The Painted Chest on the right should have a pair of Poisoner’s Gloves. You can open the door to the west with the Key you just picked up. Head through the double doors to the south and down the stairs for a scene.

You’ll walk in on Malus Thorm teaching his nurses. He’ll bid you to come forward and feel free to take in his lessons as well. You’ll the following options to respond with:

  1. I don’t wish to study, only to lift the curse on this land.
  2. A student, yes. Do please enlighten me.
  3. You will stop this sick spectacle at once.
  4. What wrong has this man done to deserve such torture.
  5. Attack.

If you choose the third option, he’ll explain that he’s actually on the brink of curing their victim. After his ode to Shar, he’ll offer to cure you to, to which you can respond with the following:

  1. [INVESTIGATION] Examine the sisters and their implements.
  2. [RELIGION] Recall what you know of Shar’s teachings.
  3. [PERSUASION] The sisters aren’t ready. They’ll make me sick instead of curing me.
  4. Attack.

Successfully investigating the nurses will allow you to tell Malus that their blades are uneven, and the nurses untrained. Malus will agree and ponder how to best train them to become steadier.

  1. [INTIMIDATION] I have read your treatise. Initiate the final lesson, or die by my hand.
  2. [PERSUASION] ‘Only with sacrifice is mastery gained.’ For the sisters to learn, you must submit.
  3. [PERSUASION] Why not have them hone their skills on each other?
  4. Attack

Failing any of these checks will result in the start of a combat engagement against Malus and the sisters. This can be a difficult fight, and they’ll all have a sizable amount of health points, so be strategic. Defeating Malus will reward you with the Surgeon’s Subjugation Amulet from his lifeless corpse. If you head upstairs and to the southern end of the hall, you’ll spot a button to activate a narrow elevator. The door leading to the library will have a lockpicking DC of 10.

Head to the left first to a Gilded Chest hidden behind the corrupted flora. It has a lockpicking DC of 10 and contains valuables and potions. Follow along the circular path to spot another Gilded Chest with the same DC containing some materials, valuables, and notably a Crystalline Lens. You can then exit out a window in the northwest corner of the room. Up on the roof, move some distance along the right and then enter the compound and to a hall-like structure to the south. Hop on the planks and then move along the wooden purchase until you find a ladder on the east side.

Down here, at the southwest area of the large room, you can find Sister Sinda. Look for a door at (X:-184, Y:11) at the east side of the chamber. Inside you can find Sister Lidwin, seemingly tending to two Tieflings. Unfortunately, if you have the Quest Save Arabella’s Parents active, you’ll recognize them to be Arabella’s mother and father. If you talk to Sister Lidwin, you can ask her why she’s treating dead people, and she’ll clarify that they’re just medicated. You can do a Sleight of Hand check to poke them and see how she reacts. The slight movement will make her think they’re alive, confirming the nurse to have already lost her wits, and that the parents are indeed dead. If you’d like to, you can purchase supplies from her though. She’ll have the following among her wares:

  • Purple Worm Toxin
  • Potion of Invisibility
  • Karabasan’s Poison
  • Wyvern Toxin
  • Remedial Potion
  • Drow Poison
  • Various other minor items

From here, you can exit out of an opening to the north, and then head to the northeast to leave the House of Healing.

 

BG3 House of Healing Notes & Tips:

  • If the Distressed Patient is still alive by the end of the fight, you can release him. He will run around until eventually despawning.

      Saw a dead goblin sharp-eye being "treated" by Sister Lidwin after the boss fight. Lae'zel insist that it's Arabella's parents. Grief strikes everyone differently, I suppose.

        One of the most memorable segments of the game for me, a shame that it's so short and there's so little to actually do here.

          There is a 21 DC Persuation option to tell the sisters to practice on Malus Thorm, killing him immediately

            The crystalline lens is not here 100% of the time; not sure if it's a random thing or what. I didn't find it in either of the gilded chests in the library. Confirmed this with several youtube vids running through House of Healing.

              So…. I was too smooth with the surgeon in my playthrough as a wizard.

              I recalled Shar’s teaching and persuaded him to let his nurses “practice” on each other.

              Then I passed DC21 check for [Wizard] [Deception] by saying to him that my magic blinded me and i need him to show me how.

              He gladly obliged and stabbed himself in the eye, killing himself.

              …. I guess thats the easiest boss fight i ever had.

                there's a DC 21 (Wizard) (Persuasion) Check after successfully recalling Shar's teachings that inspires Shadowheart and bypasses the fight completely.

                  If you want the achievement, drop some grease in front of the main doors and just swing them open to shoot some arrows or spells. Just make sure you close the doors at the end of your turn so the nurses can't use psychic attacks.

                    Depending on how you resolve the situation, you might have the opportunity to "save" the Distressed Patient by unlocking his retraints. After some tortured noises, he will get up and run away, running all around the room until he eventually despawns.

                    As of right now I have not found any follow-ups with this character. Further, I've tested cure spells, calm emotions, and lesser restoration with no effect.

                      Tested on Patch #1, August 26th 2023

                      If you opt to kill him and his nurses, several of them will drop green items with unique class abilities, however, they will not work for any of your characters. The class actions are only available for the boss to use. These include:

                      [Artificial Leech], [Trepan], [Syringe], and [Bonesaw]. The abilities that do not function for each item are "Hand me my [X], with x being the name of the above weapon.

                      They will however function as +1 weapons of the associated type.

                      Leech = Dagger
                      Syringe = Dagger
                      Trepan = Shortsword
                      Bonesaw = Longsword

                        Surgeon’s Subjugation Amulet doesnt work, unless undead zombies arent considered as "humanoid". The amulet is supposed to give a Reaction, that once per long rest when you score a critical hit against a humanoid, you can choose to spend a Reaction to paralyse said target for 2 turns. Well it doesnt do ****. Way too many items in this game dont work.

                          Holy cow, my rogue managed to convince him to let the sisters fight each other to the death, then stab his own eye out and kill himself. Also, the betting pool notice you find there... What the heck is this hospital hahaha

                            Dialog is indeed crazy, tried the different outcome and it's pretty funny but ultimately i wanted the achievement in this fight.
                            Bit tricky to do but got it.

                              I just did Malus Thorm and holy **** was this dialog crazy. With a succesful religion and charisma check (the charisma check was from my wizard class and had advantage, dc 21) i just convinced him to let him be used as an "example" meaning he was killed by his sisters. LMAO that was awesome
