      Since Patch 7 this does not drop from the Bulette anymore. Instead there is a new small secret area in the Underdark where the Kuo-Toa are worshipping the Redcap. When you are facing the altar go up on the left and you will find some mushrooms in the wall where you can jump over with torchstalks on them.
      I though for a long time that this item got patched out but usually Larian doesn't do that. Well, eventually I found it, although it is kind of well hidden.
