Spineshudder Amulet is one of the available Amulets in Baldur's Gate 3. In BG3, there are two types of Accessories, which are Rings or Amulets. Once equipped, these Accessories may give the character certain bonuses, such as new Spells, resistance against certain kinds of damage, bonus AC, an increase to an Ability Score, or more.

Spineshudder Amulet Information in BG3
- Weight: 0.05 Kg.
- Value: 65 Gp.
- Reverberation: Affected entity has a -1 penalty to Strength, Dexterity, and Constitution Saving Throws per remaining turn. When the entity has 4 or more turns of Reverberation, it takes 1~4Thunder damage and possibly falls Prone. The condition is removed afterward. Creatures Immune to Thunder damage can't receive Reverberation.
Spineshudder Amulet Location & Where to Find in BG3
- Location: Moonrise Towers (X: -169 Y: -198)
- Dropped by: Break the chest containing the mimic in one hit. It will not drop if you try to open the chest normally or fail to destroy the chest in one hit.

The amulet drop is fixed and now part of the Mimic loot.
Patch 4 notes: "Ketheric's mimic now has more loot."
Can confirm; I didn't have a way to one shot the mimic so I just killed it normally before wanting to quickload. But then the amulet was on the body of the Mimic.

Does this work on "melee" spells when used with the Distant Spell Metamagic?
Put several smokepowder bombs in a bag (or nearby basket) and throw them in to the room beside the chest. Shoot it with a Smokepowder arrow and that should one-shot the chest before it can be despawned by the appearance of the mimic (and probably kills the mimic too). Hold Alt so you can see it on the ground.

I used the 'Scroll of Disintegrate' + 'Bless' buff for best damage output on the chest.

Just to echo what others have said: The mimic does NOT drop this item, the CHEST does, and the chest disappears very shortly after activating the mimic. You have to destroy the 50HP chest before the 75HP mimic can spawn and delete it from existence.

To acquire it currently, you have to "one shot" the chest (pre-mimic), and its unavailable otherwise. The wreckage will have its own treasure trove, with the collar. A little "detonation" did the trick.

This amulet is currently not accessible in the game. The mimic at moonrise tower is supposed to drop it, but due to having the Unique flag set, it can't spawn there.
This is because it's also set to spawn in a developer room for testing item sets. They forgot to either remove it from the dev room or remove that(pointless) flag. It happens with other items in the game too.

Would probably help if this page actually explained what Reverberation is. I've not heard of this status effect before and in my playthrough I don't currently have any items that inflict Reverberation to read its tooltip.

I've looked at other sources, one of which pits this in the Mimic Chest in the Moonrise Towers (Behind guards and a locked door.) However it was not there on my playthrough. Still the best lead I have. Perhaps it's related to different stages in the game?
I came here looking for cool tips with the amulet and everyone is just complaining it was available before the first hot fix lol
