Horns of the Berserker is a Helmet in Baldur's Gate 3. Helmets can be equipped to a character's head in their designated inventory slot and provide different effects ranging from increased Saving Throws, to gaining Advantage on certain attacks.

This seems like a homage to a cut character that folks datamined before the game was released - a werewolf named Helia. The helmet is identical to the one in the datamined picture, and the description is almost like a reference to a game developer who is upset that 10 years of work was for naught.

Attack bonus works properly.
In my case helmet deals 1-4 necrotic both to you and to enemies (it can be seen even on the attack tooltip if you are not full hp). Also that 1-4 can crit on enemies for some reason, unlike other 'external' damage sources like Caustic Band (2 acid dmg to weapon attacks).
You will still lose rage charge, if you go lethargic (losing haste concentration on your caster for example). I assume you will lose it too, if you don't attack or take damage from enemies, basicly making 'taking damage' part pointless as a 'rage saving tool'. But this requires further testing.
Also, Rage: Bear heart+Bonespike Garb completly negate the self damage.

As the name implies the necrotic damage will allow barbarians to keep rage up if they don't deal/take damage.

can you still get this if you end up killing the nightsong and not having jaehira as a companion?
