If I have a flaming scimitar in my off hand and attack with it, will it give me a second bonus action to do an off-hand attack a second time?

I cannot find it, what do you mean pull the one lever? Where??

Hat gives at most 1 bonus aciton per turn. If you thought you could use Minds sanctuary to have a big turn of almost 20 fireballs, the devs thought about it

Pyroquickness Hat inflict burn, not Heat (both can be stacked, ouch).
Looks amazing with "666 Armor"

With the Elminster door open, you can grab the Pyroquickness Hat, which causes you to burn yourself if you deal fire damage with a leveled spell. However, it also grants a bonus action during that turn. The room also has a bunch of scrolls, as well as the Tharchiate Codex inside a display case.

Circlet has been replaced with "Pyroquickness Hat."
Same effect but applies burn on self as well. Still haven't found it in game, only from game files.

Since Patch 7 it can now be found in a gilded chest in the necromancers cellar during the quest "Search the cellar"
just letting you know that the page for the Blighted Village says this is found here, despite the fact it isn't obtainable for another 50 hours LMAO
