Volo's Ersatz Eye is one of the available Amulets in Baldur's Gate 3. Volo's Ersatz Eye is given to the player by Volo after his eye procedure. In BG3, there are two types of Accessories, which are Rings or Amulets. Once equipped, these Accessories may give the character certain bonuses, such as new Spells, resistance against certain kinds of damage, bonus AC, an increase to an Ability Score, or more.
While the ersatz eye is embedded in your eye socket, you can see through the tiny orb as though it were a normal eye. Can't be removed once inserted.

Confirming info that should be added to Notes; Blue 3 for those wanting to match the eye colors for their character. Wyll is not viable to receive this, or the Hag's eye for narrative reasons. Only 1 character can receive either the Hags eye, or the Erstaz Eye, not both. Even though gaining the Hag's eye gives you the option of which eye to lose. The Erstazs Eye affects the RIGHT eye. In terms of general effectiveness, it's currently affected by a bug and the effect itself can be replaced by using a Scroll of See Invisibility, which lasts until Long Rest. The bug is that after taking a long rest, the effect of the eye will vanish. When you equip or affect the character with the eye with a buff of some other kind, the effect will pop up again. The effect remains in play, despite not being visually confirmable. The save for the eye is Dexterity, as is standard See Invisibility, but the DC is based upon the class you WERE when accepting the surgery. This save DC does not change upon respeccing, but it DOES increase if the associated attribute increases as normal. The class based DC's are as follows: Fighter/Rogues/Wizards - Int, Cleric/Monk/Ranger/Druid - Wis, Barb/Sorc/Bard/Paladin/Warlock - Cha. Furthermore, Spell Saves are currently oddly situated when it comes to Multiclassing, in that, the last class level you take will override the previous one. If you take Druid to 11, your spell saves will be Wisdom, but if you take Fighter/Rogue/Wizard as your lvl 12, your Spell Saves will be Int based, even for your Druid spells. I'd need to confirm if this affects the Eye as well, but it does not seem to. It seems only to be affected by the type of Spell Save when it comes to your last selected class, and does not seem to deviate like when Multi-classing.

Dont do this with a low INT character, requires an intelligence check to see invisible creatures - which if you have the headband of intelligence isn't so bad. My dumb dumb tank is now stuck with a circlet tiara just in case X'D - DND at its finest there.
You'd think there would be an option for Wyl to at least replace his stone eye. Playing as him feels kinda gimped. But then again - there were only 3 or 4 encounters so far that could've used see invisibility, at least in Act II

I killed Volo in his cage instead of freeing him, he was annoying. However, you can loot Ersatz Eye from his corpse.

Quite sad that it doesn't give the +1 stats, would had allowed us to reach 22 stats at level 8 easy with the hag hairs, it make str character get way more benefits than others
You cannot get Volo's Eye on the same character that got the Hag's Eye. You'll have to choose a different character in your party for this.

If you got the Hag's Eye from Ethel then you can no longer get Volo's eye. Be advised.

normal game: you made a pact with a powerful mystic entity and gained the ability to detect invisible foes within 9m
larian game: some random bard failed surgery on your eye, destroyed it and gave you a magical one as a replacement

Just be aware: I had an issue where the character who had the eye was thrown off the cliff in the Underdark and was rezzed from a spectral mote, the buff for Volo's eye is now gone. Not sure if this is intended or a bug, as you resurrect with all your gear otherwise.

Keep in mind when deciding which party member you want to have the eye that when enemies make their dex save to stay hidden, its made against the intelligence score of whoever has Volo's eye (the way it would be had you simply cast the see invisibility spell). So if you're like me and, without thinking, got the ersatz eye for your big dumb unga bunga tank, its pretty worthless because enemies will essentially make their saving throw successfully every time. You can offset this if you equip them with the headband of intellect, though.

Does this eye ruin your own party member invisibility? IE if I have this lobotomy-eye on my custom character (ruining the bueno thiefling heterochromia) and then drink invisibility potion with Astarion in my party would Astarion need to roll check for it?

Fun fact: 'Ersatz' is german for 'spare' as in 'Ersatzreifen' (usually German put's together nouns that English wouldn't), which in English is a 'spare tire'.

Bastard took my good eye. I had heterochromia eyes with left one having the white part all black. I was trying to go for a theme of the Dark Urge physically manifesting in one of his eyes, while the other eye represented his good side, which suppresses the dark side. And the bastard took the good eye and left me with the evil eye. But now I can see ghosts and ninjas, so... you know.

might have been a glitch, but my Dragonborn had a permanent squint after that, wouldn't open that right eye while running around. If you like that look its great, kept distracting me so I reloaded and let my co-op partner do it instead. Drow gets a different color eye, but least they arn't constantly squinting

You can not agree to have a deal with Ethel and make operation after that to remove Hag's eye, unfortunately.
But you can have a band of three one-eyed people in your team. Maybe to the end of the game you will have full pirate crew if it's so easy to lost your eye here.

It's just an improvement, huh? Ripperdoc Volo hooks you up with a bionic eye. Though the way he kinda just tosses something so seemingly valuable at you before running off...suspicious. Obviously, you would assume it's because he realized he just mangled a proven mage/fighter's eye and wants to compensate and get outta there before he gets torn apart...But maybe there's something to it.

If you try to send Wyll to this operation he will not agree because he have only one eye left. So no free good eye for Wyll unfortunately.

I and a party member have the same build, but for some reason I have a 10% less chance to hit, can be a bug or something else. But could be the eye, not sure

this seems very OP initially, seems to be no downsides to it discovered yet, doesn't change stats anymore just...free always on see invisibility? sus...

Been changed with release or at least it has for me. It's now permanent see invisibility within 9m
After dying your character looses the see invisible buff, same as the loviathars buff.
