For anyone else having trouble getting their hands on these: Starting at level 6, both the elixir and the eyestalks get added to the pool of items that Derryth Bonecloak can carry whenever her stock gets reset. (Note: I confirm this while talking to her in the underdark, not at her shop)
For those wondering, this is the same buff that you get (temporarily) from items like Hat of Fire Acuity. It'll count down like a temporary buff until it reaches 2 turns left, but it will then reset to 3 turns each time that happens. If you gain extra stacks, *those* stacks will be temporary, but the 3 you got from this elixir will repeatedly refresh until long rest.
