Anonymousi found one in Withers' coffin. it seems like an extremely rare chance
I found a version of this but it was an arrow of Aberration Slaying. It was in the Gilded Chest at the end of the caved in trap filled catwalk in the Grymforge.
Found one outside the goblin camp in the FOREST X:-39 Y:440 on a skellington
