Us is a temporary Companion and potentially later a summonable creature in Baldur's Gate 3. Us can be found in the Nautiloid near the beginning of the game and has to be freed from Myrnath. There is a massive amount of NPC's in Baldur's Gate 3, and their ideals, needs, and way of living are strictly related to the actions they perform during the course of the game. Their attitude towards you and your party may be affected by the deeds and decisions you and your party take on.

Baldur's Gate 3 Us Dialogue Options
Your first interaction with Us is through Myrnath, in the second room of the Nautiloid during Escape the Nautiloid. Take the elevator on the left and interact with the dying elf. During the interaction, Us will be the one talking to you and asking for freedom. As you observe the brain you will have a few options
- [INVESTIGATION] Inspect the exposed brain
- [STRENGTH] Break the skull.
- [DEXTERITY] Gently prise the brain from the skull
- Destroy the brain.
- Leave.
Choose to inspect the brain and they will give you an additional option: Medicine, which requires Wisdom.
Narrator: *You notice oedema - a swelling of the brain causing pressure where it strains against the shell of the skull."
- [STRENGTH] Break the skull.
- [DEXTERITY] Gently prise the brain from the skull
- [MEDICINE] Attempt a cerebral extraction, staying mindful of the swelling.
- Destroy the brain.
- Leave.
If you successfully Succeed a Strength, Dexterity, or Intelligence check to free it. After it is freed, you will still have to make the final decision to spare the brain or mutilate it.
- [DEXTERITY] Mutilate the brain.
- Spare the creature - any injuries might weaken it.
- Spare it and it will serve as a temporary companion during the whole Nautiloid tutorial area. As soon as you land on the Ravaged Beach, Us won't be at your side anymore.
- Free Us from its cage in the Illithid Colony in Chapter 2 and you can summon it to your side in battle whenever you wish.
- If you attempt to mutilate it and fail, Us will be hostile as soon as it appears on-screen, and you won't be able to recruit it.

If you attack a neutral Intellect Devourer after recruiting Us, both Us and the Intellect Devourer will immediately attack, presumably due to a collective consciousness.

You have to mutilate the brain in the prologue so that you can meet "Us" in act 2 and get him as a reusable summon just to clarify that for anyone whos trying to figure this out.

This seems bugged on PS5. He’s not there for me despite my saving him and him surviving the fight.

i only found Us once in act 2 and i had mutilated it on the ship and chop was like “that ones all messed up so i keep it caged” or whatever. do they still show up if you don’t mutilate? cause if you dont mess up its brain what reason does chop have to cage it

SPOILERS FOR THE END:
I might have hallucinated it (too little sleep for me) but in the ending rally room arabella commented about a kitty which i assume was us cause i had them summoned and dont own a cat.

I swear Us was my good buddy Helm Friend 100+ hours ago, and it definitely survived the Nautiloid, but Chop's cage is empty and there is no Us for me in game now :(
Don't know what I did, or if it's a bug.
Initially I thought it was Us dead on the beach from the crash, so I kept what I realised was just a random Intellect Devourer next to Withers in camp, as if to say "revive him, then, bone man" as I wanted Us back.
But later I learned he might pop up again, been looking forward to the reunion, but game says nope.

Well, snap. My lobotomized Us died killing Commander Zhalk. I had to sacrifice him (her, it) in order to finally manage to kill the Commander on tactician. He was lobotomized anyway, so he is probably better off dead anyway.

It doesn't die in the nautiloid, you can find it again after jumping after Ketheric, in the room where Chop is, in a cell.

Us tells us that when travelling it has a "Kitty" form for all but my closest companions so it doesn't draw attention. I wish we could ask it to take on that form even for us, I want a cute kitty following me around.

looks like the lobotomy doubles Us' health
when i just pluck the brain and let it go it has 4 hp, if lobotomized it has 8

"Lobotomise" reduces Us' saving throws under Intelligence and Wisdom. This is only a disadvantage .

Don't take the lobotomy option when first encountering Us. There's no benefit if you succeed at the check, and if you fail you lose a useful ally.

I was happy to see Us later in the story. I didn't kill (obviously) or damage Us during the intro

That is not the full information, if Us survives first act you can Find him Later in Third Act in the Mind Flayer Colony - Morgue inside a cage. It gives you then an item and you can use it to summon Us at will.

Not sure if this is related to whether or not you choose to mutilate Us, but In the finale of Act II you can find Us again and acquire them as a permanent summon item. When summoned it lists "Lobotomized" as one of its conditions, but it doesn't say what effect this has, if any.

Idk it's b/c some of my mods or not, but the Us somehow has more HP when I successfully mutilated it. (17 without mutilating, 21 with mutilating)
