Myrnath is an NPC in Baldur's Gate 3. Myrnath is a dying high-elf on the Nautiloid with his brain exposed. He was infected with a parasite that led to an Intellect Devourer to take over his brain.

You can choose between:
- Who am I talking to - a man or a brain?
- "A newborn. Born new from this husk"
- I think you're past the point of saving. Tell me what to do.
- "Remove us from this body - from this case free us. Please!"
- Investigation [Skill Check: 10]: Inspect the exposed brain.
- Strength [Skill Check: 10]: Break the skull.
- Dexterity [Skill Check: 10]: Gently pry the brain from the skull.
- If you are successful in removing the brain from the skull, you can use Dexterity [Skill Check: 15] to mutilate the brain and add the aberration as a companion. If you fail this check, the aberration will turn hostile once freed.
- You sound afraid. Why?
- "The enemy. So many enemies."
- Destroy the brain.
- Leave

The page says Us will be hostile if you don't mutilate it. That's not true, he joins you either way.

The info about mutilation is a bit misleading, since it doesn't turn hostile if you don't even attempt to mutilate it. I think last entry in 2 dialogue option should be changed to something along those lines:
"If you are successful in removing the brain from the skull, you can add the aberration as a companion. Before it joins you, you can use Dexterity [Skill Check: 15] to mutilate the brain, making it possible to attack non-hostile Intellect Devourers (there's only one you'll encounter on the ship) without turning hostile, at the cost of lowered statistics & HP. If you fail the mutilation attempt, it will turn hostile."
Right now it reads as if without mutilation it will get hostile, which is not true - it only does when you fail the DEX Skill Check to lobotomize it.
IMHO you should either forgoe the mutilation attempt or kill it on the spot.
