Bhaal is a Deity in Baldur's Gate 3. Deities are worshiped all along Faerûn, and one must be selected once you create a Cleric. Deities impact how NPCs react to your character and how some events may unfold differently based on the God you worship. There is no other gameplay implication depending on the Deity chosen.
Bhaal is the notorious god of murder, reborn after each of his descendants were viciously slain. He has a tenuous alliance with Myrkul and Bane, the three having attained divinity in a bargain with the fallen god Jergal
In Baldur's Gate 3, Act 3, you may be able to find a temple dedicated to Bhaal in the city sewers. One way to open the door to the temple is by shooting the corpses hanging down in front of the door, bleeding them over the skull.
