Sneak inside of her house. Place a character or two inside of her chimney by the spot she teleports to when she is trying to run away to block it, then come in through the front door with the rest of the team for the conversation prompts, If you engage her coming through the chimney she would start the fight and skip the conversations which will cause the big chest outside to bug and it will never unlock even after you kill her.
When she tries to teleport away, she will be blocked by the characters at the chimney and can't make it downstairs. Now you can continue the fight with her inside the house.

I haven't tried it but I guess you could also place barrels in her way instead of characters.

I decided to figure out a way to kill her at the tea house in the swamp.

Step 1) Accidentally learn she is scared of spiders
Step 2) Use Gale's "Find Familiar" or my druid's wild shape spider.
Step 3) Walk towards her, she keeps a certain distance because she is arachnophobic.
Step 4) Walk her out to her porch, pushing her to the very edge of it
Step 5) Have your 3 remaining party members in a beatdown circle at the bottom of the porch
Step 6) Blast her off the porch with a knockback (I used Gale's Thunderwave) into the middle of everyone -- no matter where she turns, she'll get opportunistically clapped in the ass. She will habitually try to flee, like a coward.
Step 6) Destroy that child experimenting old hag

I knocked her out in turn 1 in her lair. I had non-lethal still on my passive and Laezel just knocked her. I only took 10 health off her max.

The Sussur Bark-infused weapon you can get from the forge in the Blighted Village would be perfect for this fight, as it silences on hit, but in the first fight (before she vanishes and hides in her lair) I had her Silenced, Prone, and Frightened on Turn Two even after she tried invisibility… Got up and poofed anyway.

Thank you, Larian, for being a lousy GM. If I’ve built my characters to stack statuses on a boss and it’s not immune, I should be allowed to stun it to death.

Interesting enough, this seems to be built off the One Shot called The Lair of Auntie Ethel. https://www.dmsguild.com/product/235060/OneShot-The-Lair-of-Auntie-Ethel

I am wondering if there are multiple one-shots built into the game and what that means for copyrights....
